APPLICATION FOR THE INSTALLATION OF NEW WATER SERVICE CONNECTION

 

This Service provides guidance to residents of Plaridel who would like to apply for a new water service connection to PLAWD.

This Service is open to all residents of Plaridel.

1. Community Tax Certificate (Cedula) Municipal Office of Plaridel or Barangay office
2. Sanitary/Plumbing Permit Municipal Engineering Office of Plaridel

3. Original and photocopy of valid Government-issued I.D. of the applicant:

a. Driver’s License

b. SSS UMID Card

c. Passport

d. PRC License

e. Senior Citizen ID

f. PhilHealth ID

g. Voter’s ID

h. Postal ID

4. Proof of ownership – any of the following:

Land Title, Tax Declaration of the Property, Official Receipt of Real Property Tax for the current year.

5. If the applicant is not the property owner, an authorization letter from the property owner to apply water service connection for the property. From the property owner

8. The applicant must attend the seminar for the new concessionaires held every Monday to Friday 8:00 a.m. - 4:00 p.m. At Plaridel Water District (PLAWD) Office

2. Check correctness of data in the Contract and sign if it is correct. 2.1 Check if documents are complete and properly-signed. 2.2 Request client for a sketch of their location or vicinity. 2.3 Inform client of the schedule of inspection and seminar. 2.4 Advise client to pay the corresponding fees at the Cashier. 3. Present application form and pay the corresponding application fee. 3.1 Accept payment and issue Official receipt. 3.2 Forward all documents to Customer Service for scheduling of site inspection. Fixed fee for the Application Fee: PhP150.00 3 minutes Cashier

5.
