Temporary Service Disconnection
This service involves concessionaire’s request to remove the installed water service, provided the concessionaire pays the outstanding water bill. The client needs to sign the service request form and pay in full payment of any outstanding water bill.

2. A copy of the Certificate of Title of Land. If the applicant is not the owner of the lot, furnish a request letter signed by the owner. | Own copy or a copy from the owner of the land. |
